Title(s) and abstract

Scientific title Performance of colposcopy in the diagnosis of histologic cervical intraepithelial neoplasia 2+ (CIN2+) in Bhutan: a multi-centre cross sectional study.

Background In Bhutan, cervical cancer is the most common gynaecological cancer and leading cause of death from gynaecological cancers. HPV DNA testing has replaced conventional pap smear in 2020 for the national cervical cancer screening method. In cases of positive HPV-DNA result, colposcopy is an important procedure where it helps in localising the anatomical site of suspicious lesions. Then biopsies are taken for confirmation of diagnosis by histopathology examination. Several quality improvement initiatives on colposcopy have been undertaken in Bhutan. In this study, we aim to assess the performance of colposcopy in the diagnosis of histopathologic CIN 2+ in Bhutan.
Objectives The aim of this study is to assess the performance of colposcopy in the diagnosis of histopathologic CIN 2+ and margin positivity of LEEP specimens in Bhutan.
Study Methods This will be a cross-sectional study conducted across Bhutan in seven colposcopy centres namely Jigme Dorji Wangchuck National Referral Hospital (JDWNRH), Eastern Regional Referral Hospital (ERRH), Central Regional Referral Hospital (CRRH), Trashigang Cluster Hospital (TCH), PGH, Lungtenphu Army Hospital and Wangdue hospital. Sociodemographic data will be collected using a standard pro forma; information on HPV-DNA and histopathology results will be extracted from the Department of Pathology, JDWNRH, ERRH and CRRH using the unique citizenship identity card number of study participants. The colposcopy details will be directly entered by the colposcopist at the time of colposcopy. Ethics approval will be sought from the Research Ethics Board of Bhutan; administrative approval from the Ministry of Health and the Hospital Administration from the respective hospitals. Data will be entered into EpiData 3.1 (Odense, Denmark) and will be analyzed using STATA 13.1
Expected outcomes and use of results The socio-demographic and clinical variables of the participants will be presented. Sensitivity, specificity, positive and negative predictive values of colposcopy and the margin positivity of the LEEP specimen will be presented.
